      Instructions:

      To request changes to products for your location(s), please provide the following information in the form below:

      • Impacted location or location groups
      • Products to be changed

      Expectations: 

      dbaPlatform will complete this request within 2 business days, provided no issues are encountered by our partners. The requested updates are estimated to show live within 2 business days after dba completes the request.
